Diversability Advocacy Network - DAN
Our Flagship Systems Change Coalition
Formed in 2012, DAN serves the Northern Sacramento Valley and surrounding mountain counties in partnership with the Disability Action Center, Passages, Far Northern Regional Center, and the North State Office of the State Council on Developmental Disabilities.
DAN’s primary focus is to serve as a trusted and reliable rural-county source of information regarding long-term services and supports (LTSS), and legislative and health care changes affecting all age groups.

Concerns about homelessness in Shasta County continue to grow year after year. As a community, we've taken notice of the social and financial impacts of this issue. While the solutions are diverse, the desire for urgent and effective action is shared through out the community.
The total number of homeless people who were counted in Shasta County jumped 19.5% during the 2022 annual Point-in-Time Count of the Homeless population.
Studies suggest that this population could actually exceed 3,000 every year.
WHO WE ARE
Project Homeless Connect (PHC) is a community-based effort with the goal of connecting those either experiencing homelessness or at risk of experiencing homelessness with essential services. PHC is accomplished through a collaboration of numerous community partners with a shared focus on making the most of the resources we have available. This event is designed to bring service providers together under one roof to allow for more effective coordination of services.
OUR MISSION
To remove barriers and provide access to long term solutions that will help people move towards a more sustainable life. In collaboration with other service providers, we believe we can make a positive impact in health and vitality of our community.
OUR SERVICES
Our goal is to reduce the financial impact of homelessness of our community by providing the wrap around services needed to assist people into stability. PHC will host a variety of opportunities that will provide a hand up to our community members experiencing homelessness. These services range from housing, employment readiness services, DMV, vital statistics, health care, vision, family planning, mental health, substance abuse, domestic violence, disability, case management and many other entities to assist people with engaging in the support needed to reduce housing barriers and move people towards housing options and stability. Bringing essential resources together in one location provides a more thorough opportunity for individuals to connect and engage.
